Team Members
CCC’s team members pride themselves on building strong, caring relationships with each child and their family. This personal touch allows case managers to fully understand the medical, emotional and social needs of each patient and their family. The team includes a medical director, program director, administrative assistant, educational consultant, and case managers that are nurses or social workers.
