Case managers (also called care coordinators) coordinate the health care, community support services, and resources for children with special health care needs.
Case managers act as advocates for patients. Their coordination extends outside of medical facilities.
Our case management team is comprised of social workers, nurses, and certified case managers. Our case managers work closely with clinical social workers at the Children's Hospital of Richmond to determine priority needs for your family.
Case managers are able to contact the personnel at schools, hospitals, property management companies (landlords), financial offices (Medicaid), and insurance companies on behalf of patients and their families.
Our care coordinators often solve problems such as arranging Medicaid-paid transportation to and from appointments, reminding families to renew their Medicaid benefits, navigating individualized education programs at a child's school, and locating accessible resources for families.
Yes. CCC offers free services to eligible children in the state of Virginia.
CCC services are paid for by the Virginia Department of Health. Services are free to your family, so you do not need to be insured to enroll.
CCC does not offer direct financial aid. However, care coordinators can help connect you to financial resources in the community or through the hospital.
In rare cases, uninsured patients can receive financial aid from our pool of funds to cover the cost of medical equipment or prescription medications. Click here to learn more about the pool of funds.
CCC works collaboratively with VCU, but children do not need to be patients of VCU to qualify. Many children see doctors at Bon Secours hospitals and other doctors in the community. Case managers can help connect a child to appropriate medical care within or outside of their established hospital network.
