Community Connections

While each Care Connection for Children program is funded and administered by the Virginia Department of Health, each program is locally managed, offering a community-based approach to care. Members of the staff understand the needs of the children and families they see each day. 

Professional care coordinators work closely with children and their families to provide targeted, comprehensive and culturally appropriate care. These professionals coordinate all services offered by the various providers to children in need, and they actively seek input from families and medical practitioners.

  • Referrals

Our care coordinators connect families with

  • Local health care providers who have expertise with children with special health care needs.
  • Resources from the community and referrals to other community services
  • School programs and services.
  • Other families who are experiencing similar situations.
  • Ongoing help in coordinating specialty care and accessing services.
  • Assistance in obtaining health insurance and using health insurance benefits
  • Specialized health insurance programs.
  • Help in determining eligibility for access to a “pool of funds” to pay for services, including medical equipment and medications.
